COM/COL Form

This form can either be sent electronically or printed and and mailed in. If you are sending electronically please upload a scanned sample of your material where indicated below or return a separate form with each piece of your COM shipment to:

If material is being sent from a source other than the purchaser, that source should insure that the information requested on this form is provided. Proper identification of COM is imperative. If we are unable to identify a COM, the order cannot be processed. Orders using COM/COL cannot be processed until all COM/COL has arrived at our factory.

Notes:
In the absence of application instructions, Councill Contract will not be held responsible for a COM application deemed unsatisfactory by the customer.

While we do inspect fabrics for mill imperfections, we cannot be responsible for defects, color or dye lot variations, or other flaws in fabrics.

Councill Contract accepts no responsibility for a fabric's overall appearance, flammability, normal durability or color fastness.
